Mission Statement
The mission of the Orthopedic and Sports Physical Therapy Center is to provide the highest quality physical therapy services from the most technically trained staff in an environment that maximizes patient/therapist contact and customer service.Our most frequently treated diagnoses:
- lumbar sprain
- cervical sprain
- rotator cuff tendinitis/bursitis
- osteoarthritis (neck, back ,knees ,hips, shoulders)
- tennis elbow
- carpel tunnel
- hip bursitis
- knee sprain
- ankle sprain
- status post surgery ( total hip replacement, total knee replacement, ACL reconstruction, rotator cuff repair)

Here are the links to download our Patient Information Sheet and Prescription. Take the Prescription form to your doctor for a referral to Physical Therapy. Then, you can download the Patient Information Sheet, fill it out, and fax to any of our locations. We will call you within 24 hours to schedule an appointment!
